 The Affordable Care Act hopefully will hopefully improve access and quality of care for the US Aging population as well as high risk groups that heretofore have not been able to manage their chronic health conditions. Risk sharing among health professionals that addresses outcomes and meeting quality indicators are been imbedded by payers as well as CMS for the Medicare and Medicaid population. Setting up the need for population health management in order to keep patients healthy and out of the hospital and the ER were addressed through many of the innovative technology offerings as well as communication linkages from patient centric solutions such as EHR ‘s as well as patient, provider and caregiver portals.

 Many of the health care content sites as well as self care interventions that are on line and can be kept on your body are targeted from management and self management tools that hopefully will focus on prevention and chronic care needs of everyone that will manage their risks over time. The provider and how they organize care through Accountable Care Organizations will be delivery systems that we need to monitor to gauge ROI and cost benefits vis-à-vis Medicare Advantage Plans as well as the growth of Managed Medicaid platforms. We heard and hope that technology can be the key to efficiently identifying and treating populations.
